when an employee makes a mistake you bring corrective action, if they make the same mistake again and its obvious they have not learnt from the first mistake then its time to consider if the person can actually do the job.
when you keep people in a job who continue to screw up over and over then you deserve what you get, your business will fail because of bad management.
