Questions 1 and 2 have been answered...

On Question 3 it looks to me that you have been charged the exchange fee which is a very small fee charged to the seller on the value of the transaction. I note that it is not charged by my Canadian broker as it seems to be absorbed in the commission, but all my US brokerage accounts subtract that fee (dependant on value) from all sales. It's only a few pennies unless you are trading in million dollar lumps....
