1099 Form Crystallex Delisted

Has anyone had an issue with their 1099 showing that Crystallex has been delisted? What is annoying is in my unrealized gain/loss for Ameritrade it still shows the shares I have owned and Crystallex. Do I have to reach out to Ameritrade and say that it wasn't delisted but it still in legal hell forever?

Has anyone dealth with this or have any suggestions?
