same happened to me. I called my broker Monday after the first tender deadline and tendered on a best efforts basis. (monday at 2:00 was my brokers cut off time)

I just called them again today assuming that because the tender deadline was pushed to the 16th that my original tender vote would now be filled. My broker explained that the best efforts tender vote was rejected on Monday and that I needed to vote again today. I did re-tender today but if I had not called my shares would not have been tendered. I had to re-vote for the new tender deadline, the original instructions did not carry forward to the extended date.
