I think it's a case of this, and the "bug" is in a human being at the brokerage.

have we ever seen a false insider buy flag?

I don't think so, only the manipulatively false insider sell flags, typically written off as just simple broker "mistakes".

given that, it is rather easy to draw one's own conclusions about what the "certain circumstances" are.
