I think the most plausible course is the one suggested by Dr. Kausik Ray, a new trial with something like 5,000 patients and 500 Events with CFH replacing stroke.  Call it "BETonMACE II - This time its Chronic".  

That still leaves the billion dollar question of how it would be paid for or by whom.
