Healthcare costs sky rocketing?

My wife had a bone density test done last month. It's non invasive...simply a scanning process.
We just got a Horizon BC/BS notice of the final cost. The hospital billed $1833.00. Medicare "allowed" $30.88, which was paid by BC/BS.
This is interesting. I gotta believe the correct amount is somewhere in between. I have no idea where, because I'm not privy to the cost vs a reasonable profit margin. Any docs here who want to respond? I'm not looking for a debate....just answers.
Another thing....my insurance covered that nominal amount of $30.88. Why not ask me to pay something like this relatively insignificant amount and maybe the premium could be reduced? It would be like taking a higher deductible on you car to save $ on the premium. After all, insurance should be purchased for catastrophic events not indemnification from dollar one.
That's just one idea. There must be others which would bring down the costs both in Medicare and the private companies.
