G4Gold, I'm unsure of the purpose of your reply to my previous post.  I only asked whether this form needs to be corrected if the verbage regarding the power of attorney on the 10-K is incorrect (since Tracy said emphatically that there is no power of attorney granted). 

I will take her at her word, which means that this wording on this legal document must then be incorrect.  If it is incorrect, then should it be corrected so as to avoid legal liability and confusion?
