Perhaps some of these reported "violations" are a simple matter of clicking the report violation button instead of the next message button which is directly below. I know I have once clicked a violation button by accident and had to retract the violation when prompted. I have heard at least one other poster on this forum say the same had happened to him. I think it is especially a problem when using a mobile device such as a Blackberry.

Just a thought why there may be some violations reported with no coroborating evidence.

Who would do that?
