Good points bought up here Chappy. And all three cases blatant human decisions made. Well especially the gulf incident. Some say there are no accidents but miscalculation and/or human error. There has been over these cases a reform in proceeder's for these operations . After the gulf mistake made by the rep for BP, the fines incurred for that would be enough to deter someone for making this shortcut again. One has look at how stupid it was when you look at operations in the North Sea operations. That is challenging at the best of times. But yet successful in safe operations.
