I have always thought it was a middle eastern country since there are a few of those which provide health benefits at no cost to all of the citizens. I am not so sure population would drive the purchase. Several of the countries that produce oil have the highest per capita income in the world. The problem is the money is concentrated in the hands of a very few. Money and benefits are distributed in these countries to keep the plebes from uprising.
